Referring Physicians

Thank you for referring your patients to Kleinert Kutz Hand Care Center. Below is contact information for scheduling new and return appointments, as well as a link to a Referral Fax Form. Please complete the Referral Fax Form with the patient’s information and a direct number back to your office. We will call the patient, schedule the appointment and then fax you back the appointment confirmation. If you do not prefer to use the fax sheet, please feel free to call the schedulers directly to make an appointment.
